 Address book won't insert e-mail address

I'm using Outlook 97 on Windows 95.  When I use the address book to
compose a new message the correct Contact list is displayed but the
e-mail address in parenthesis is just generic.  An example would be
"Stan Martin (E-mail)", or "Suzanne Somers (E-mail2)".

It appears that somehow the field name is being inserted instead of the
contents of the field.  So far I have:

Run "Nickname.exe" but immediately got an error message stating "Error
Reading Nickname Cache".
Exported the contacts.pst file to an .mdb file and manually deleted all
values in the "Display Name" field.  Then imported the .mdb file back
into outlook without the "Display Name" field.
I have completely uninstalled Outlook and removed all profiles and
services.
Re-installed the program and created new profiles, and services from
scratch.
Run Scanpst.exe to correct any errors.  Errors were found and were
repaired.

I had never used outlook before today and it has been a long day.

Any suggestions are greatly appreciated

If an address is resolved, you'll always see the familiar name placed in the
To: field. If you right click on the name in the To: field, do you not see
the actual address in Properties?
